The country%27s journey toward elimination has been marked by phased progress%26mdash;early local success, national scale-up of interventions, and periodic resurgence.%3C/span%3E%3C/p%3E%0A%3Cp class=%22pf0%22%3E%3Cspan class=%22cf1%22%3EVanuatu%26rsquo;s first elimination success was achieved on%26nbsp;%3C/span%3E%3Cspan class=%22cf1%22%3EAneityum%3C/span%3E%3Cspan class=%22cf1%22%3E Island in %3C/span%3E%3Cspan class=%22cf1%22%3ETafea%3C/span%3E%3Cspan class=%22cf1%22%3E Province in the early 1990s through a combination of mass drug administration (MDA), insecticide-treated nets (ITNs), and strong community-based surveillance. Although malaria was interrupted by 1996, a P. vivax outbreak five years later highlighted the island%26rsquo;s high receptivity and the risk of resurgence%26mdash;lessons that continue to shape the national strategy.%3C/span%3E%3C/p%3E%0A%3Cp class=%22pf0%22%3E%3Cspan class=%22cf1%22%3EBuilding on%26nbsp;%3C/span%3E%3Cspan class=%22cf1%22%3EAneityum%26rsquo;s%3C/span%3E%3Cspan class=%22cf1%22%3E experience, the Ministry of Health (MOH) selected %3C/span%3E%3Cspan class=%22cf1%22%3ETafea%3C/span%3E%3Cspan class=%22cf1%22%3E Province in 2008 as the pilot for provincial-scale elimination. %3C/span%3E%3Cspan class=%22cf1%22%3ETanna%3C/span%3E%3Cspan class=%22cf1%22%3E Island, the most populous in the province, was the focus of intensified efforts including indoor residual spraying (IRS), larval source management, rapid diagnostic tests (RDTs), and community mobilization. These interventions led to the last locally acquired case in 2014 and WHO certification of malaria-free status for %3C/span%3E%3Cspan class=%22cf1%22%3ETafea%3C/span%3E%3Cspan class=%22cf1%22%3E in 2017. %3C/span%3E%3Cspan class=%22cf1%22%3ETanna%3C/span%3E%3Cspan class=%22cf1%22%3E became a national model, demonstrating the feasibility of elimination with sustained leadership, investment, and partnership.%3C/span%3E%3C/p%3E%0A%3Cp class=%22pf0%22%3E%3Cspan class=%22cf1%22%3EIn 2021, the government launched the National Strategic Plan for Malaria Elimination (2021%26ndash;2026), aiming to progressively eliminate malaria province by province. However, from 2022 to 2024, malaria cases resurged in northern provinces%26mdash;particularly%26nbsp;%3C/span%3E%3Cspan class=%22cf1%22%3ETorba%3C/span%3E%3Cspan class=%22cf1%22%3E and %3C/span%3E%3Cspan class=%22cf1%22%3ESanma%3C/span%3E%3Cspan class=%22cf1%22%3E provinces.%3C/span%3E%3C/p%3E%0A%3Cp class=%22pf0%22%3E%3Cspan class=%22cf1%22%3EThe targeted response resulted in control of the situation in%26nbsp;%3C/span%3E%3Cspan class=%22cf1%22%3ETorba%3C/span%3E%3Cspan class=%22cf1%22%3E but %3C/span%3E%3Cspan class=%22cf1%22%3ESanma%3C/span%3E%3Cspan class=%22cf1%22%3E has continued with the rising incidence. Additionally, early 2025 has seen continued sharp increases in %3C/span%3E%3Cspan class=%22cf1%22%3EPenama%3C/span%3E%3Cspan class=%22cf1%22%3E and %3C/span%3E%3Cspan class=%22cf1%22%3EMalampa%3C/span%3E%3Cspan class=%22cf1%22%3E provinces, with Shefa province reporting high numbers of imported cases, both domestic and international. These trends indicate that Vanuatu is falling off-track from its elimination targets and requires renewed focus and strategy.%3C/span%3E%3C/p%3E%0A%3Cp class=%22pf0%22%3E%3Cspan class=%22cf1%22%3ETo address this, the MOH, with technical support from WHO, has initiated the development of the Vanuatu Malaria Elimination Framework (VMEF) 2026%26ndash;2032. This upcoming strategy adopts an island-based model to elimination, which tailors%26rsquo; interventions to the geographic, ecological, and transmission characteristics of each island group%26mdash;building on lessons from%26nbsp;%3C/span%3E%3Cspan class=%22cf1%22%3EAneityum%3C/span%3E%3Cspan class=%22cf1%22%3E and %3C/span%3E%3Cspan class=%22cf1%22%3ETanna%3C/span%3E%3Cspan class=%22cf1%22%3E while addressing operational challenges faced in larger, more mobile populations.
